If for any reason you are unsatisfied:

• Write the 5 digit UPC code from the back of the can on your receipt. For reference: 10163 for Chocolate, 10164 for Vanilla, 10165 for Strawberry, 10167 for Banana, or 10168 for Strawberry Banana.
• Write the reason for the requested refund on the back of your receipt.
• Mail the receipt to the following address:

You will receive a reimbursement check directly from Dairy Farmers of America (DFA). DFA holds the right to confirm your identity. Incomplete or late requests will not be honored or refunded.
